I fly regular missions to the ISS and beyond in Orbitersim (http://orbit.medphys.ucl.ac.uk/) the realism can get a bit mind boggling sometimes...
Lately though I've been helping the Kerbals get into space (http://kerbalspaceprogram.com/)

The program is still in active development and a little rough around the edges but incredibly addictive designing your own space rockets and then watching them blow up on the pad!
